Miles of Smiles will be coming to Berkeley School District 87 in February and March 2024. All children who sign up are eligible to receive a dental exam, prophy treatment, fluoride varnish treatment, and dental sealants.
This program is FREE to the school and parents. This dental exam will fulfill the mandated dental exam for K, 2nd, 6th, 9th Grades. After we see your child at school, a paper will be sent home telling what services were provided and if further dental treatment is needed.
Signing up for dental services at school is as easy as scanning the below QR code. Once scanned, create an account, provide your child’s information on the fillable form & sign using DocuSign. You are able to save a copy of the signed form too.
